LIABILITY WAIVER
Parent/Guardian acknowledges that St. John's and their directors, teachers, volunteers & representatives, are NOT liable for:
Parent/Guardian, on behalf of the child, heirs, executor and assigns, I hereby:

• (A) WAIVE, RELEASE & DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for personal injury, property damage, property theft, or actions of any kind which may hereafter occur to my child.

THE FOLLOWING ENTITIES OR PERSONS: The Diocese of Fargo and St John the Evangelist Catholic Church and /or their directors, volunteers, teachers & representatives.

• (B) INDEMNIFY, HOLD HARMLESS & PROMISE NOTTO SUE the aforementioned entities or persons from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

PARENT/GUARDIAN, ON BEHALF OF THE CHILD, HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH THIS EVENT, including by the way of example and not limitation, any risks that may arise from negligence or carelessness on part of the persons or entities being released from dangerous or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I grant permission to St. John the Evangelist Catholic Church for the use of my child's name, likeness, and/or photographic image to appear in parish publications and/or website/social media sites.  I understand that if, for whatever reason, at any point in time, I decide to revoke this agreement and I so notify the parish in writing, all references my child's name, likeness, and/or photographic image will no longer be used.  I understand that website/social media references and website/social media photographic images will be removed within thirty (30) days of the written notification.  I understand that the parish is not responsible for access to the information or downloads made by users using websites or social media sites prior to this removal of references (i.e. name, likeness, and/or photographic image).  I further understand that my child's name, likeness, and/or photographic image may continue to be used in any publication already printed or published prior to my revocation of the consent.
